Hand & Wrist Pain: Self-Tests & Specialist Access in Sweden

Medically reviewed by specialist physiotherapist · Updated 11 October 2026 · Vardse Nordic AB

Nocturnal hand numbness, tingling in the fingers, or sharp pain at the base of the thumb during everyday pinching tasks can disrupt professional performance. For expats and international residents in Sweden, understanding whether symptoms originate from carpal tunnel syndrome, thumb basal joint osteoarthritis, or tendon sheath inflammation is vital for timely intervention.

Three Guided Orthopedic Self-Tests

Perform these functional tests gently while seated.

Test 1: Phalen's Test (Carpal Tunnel Syndrome)

Procedure: Press the backs of your hands firmly together with wrists flexed at 90 degrees and fingers pointing straight downward toward the floor. Maintain for 60 seconds.

Positive Finding: Numbness, tingling, or tingling sensations developing in the thumb, index, or middle finger within 60 seconds indicates positive carpal tunnel median nerve compression.

Test 2: Finkelstein's Test (De Quervain's Tenosynovitis)

Procedure: Tuck your thumb inside your fist and wrap your four fingers tightly around it. Slowly bend your wrist downward toward your little finger (ulnar deviation).

Positive Finding: Sharp lancinating pain over the radial aspect of the wrist confirms inflammation in the thumb extensor tendon sheath.

Test 3: Grind Test (Thumb Basal Joint Osteoarthritis)

Procedure: Grasp the base of your thumb and push axially straight into your wrist while gently rotating in small circles.

Positive Finding: Grinding sensation or acute pain deep at the base of the thumb confirms CMC-1 joint degeneration.

⚠️ Urgent Clinical Warning: Permanent Nerve Loss

If nighttime finger tingling progresses into permanent daytime numbness or if the muscular pad at the base of your thumb begins to flatten (thenar muscle atrophy), urgent hand surgical consultation is required to decompress the carpal tunnel before nerve damage becomes permanent.
